Facial Fillers to Treat Wrinkles and Fine Lines

BUFFALO, NY - This is one of the fastest growing segments of cosmetic surgery. The immediate correction of fine lines and wrinkles is one of the most gratifying treatments a person can experience. With the increasing desire for people to achieve a more youthful appearance there is a great attraction for treatments that offer immediate results with little downtime the so called "lunch time procedures".

The first injectable dermal filler was approved by the FDA in 1981. Bovine collagen under the name of Zyderm was the only filler available for over 20 years. Skin testing of the Zyderm was required to rule out any allergy since the bovine protein collagen is different than human collagen. The addition of lidocaine to the product allowed an almost pain free application. Zyderm had limited application for deeper rhytids so Zyderm II and Zyplast were released to address these shortcomings. Probably one of the major drawbacks to these products was their short longevity. Generally, these products lasted only 3 months.

The pharmaceutical community recognizing the need for longer lasting dermal filler has responded since the year 2000 with an abundance of products. The goal has been to develop dermal fillers with the following characteristics: hypoallergenic, safe, a natural feel, inexpensive, painless to inject, long lasting, low risk of complications, minimal downtime and with consistent and predictable results.

The list of fillers is growing at a rapid pace with no end in sight. Some of the products available presently include: Restylane, Perlane, Juvederm, Hylaform, Captique, Puragen, Prevelle Silk, Artefill, Artecoll, Sculptra, Radiesse, Evolence, Hydrelle, Elevess. My staff and I have evaluated many of these products and will be able to guide you in selecting the best product to correct your concerns.

Some of the most popular products today contain hyaluronic acid. Hyaluronic acid is identical in all species and the risk of allergy is remote. The first hyaluronic acid filler or HLA to gain FDA approval was Restylane in February of 2003. This product achieved excellent long lasting results with minimal adverse effects. The next HLA to appear on the scene and receive FDA approval in 2006 was Juvederm. Juvederm is similar to Restylane but is produced with more cross-linking of the molecules improving the longevity. Other HLAs include Hylaform, Puragen, Prevelle, Elevess and Hydrelle. Some of these products contain Lidocaine making for a more comfortable treatment but at the present do not appear to last as long as Restylane or Juvederm.

Radiesse is a novel filler approved by the FDA in December of 2006. This product is composed of calcium hydroxylapatite and a carrier gel. This product is injected a little deeper than HLAs just below the skin. It can be mixed with Lidocaine to allow for a more comfortable treatment. An additional benefit is that it can be used for soft tissue augmentation.

Considerable advances in technology have given the cosmetic surgeon a plethora of filler options. Still no one filler can address every patient's concern. Treatments are tailored to the patient's individual needs to achieve maximal benefit, minimize risk and achieve the desired correction longevity.
